Please fill out information below.

*As it is very important for us to maintain the credibility of this registration as well as the artists involved, we will require one form of verification listed below to enter the Artist Registry. Please send one of the following documents to katie@puebloarts.org with the subject of "REGISTRATION VERIFICATION". Once verification is received, your entry will be listed on the registry within 7 business days.

Copy of certificate of Good Standing with the Secretary of State.
Copy of your business license.
Reference Letter.

I give permission for the Pueblo Arts Alliance and the Creative Consortium of Pueblo to display my entry onto their Artist Registry on the Pueblo Arts Alliance website.
